Fred Hutchinson Cancer Center is an independent, nonprofit organization providing adult cancer treatment and groundbreaking research focused on cancer and infectious diseases. Based in Seattle, Fred Hutch is the only National Cancer Institute-designated cancer center in Washington.

The Community Health Worker (CHW) serves as a Lung Health Navigator, combining outreach, engagement, and patient navigation to improve access to lung cancer screening for individuals at high risk. While supporting all community members, this role places a special emphasis on populations disproportionately affected by lung cancer, particularly American Indian and Alaska Native communities.

As part of a collaborative team, the CHW identifies people at high risk for lung cancer, determines eligibility for screening, and provides navigation support to coordinate screening and follow-up across community clinics, Fred Hutch Cancer Center, and partner organizations. Key responsibilities include outreach and navigation support to help individual address barriers to screening and ensure timely follow-up to support optimal care.

Responsibilities

• Assists with implementation, management and tracking of a Tobacco Use survey.
• Assists with patient navigation to appropriate Fred Hutch smoking cessation programing and lung cancer screening, and to community-based resources as needed.
• Provides smoking cessation counseling and associated resources to community members.
• In collaboration with the Fred Hutch American Indian/Alaska Native Patient Navigator, assists in culturally appropriate patient / family transitions between community-based care and Fred Hutch.
• Serves as a cultural broker/representative among Fred Hutch clinical and həliʔil Program staff, maintaining ongoing communication with affiliated programs and institutions, while representing the həliʔil Program to community partners and Indigenous/Tribal organizations.
• Fulfills and maintain all reporting requirements in accordance with Fred Hutch and həliʔil Program policies.
• Maintains metrics as per həliʔil Program guidelines, and identifies trends/gaps.
• Participates in staff meetings, activities and trainings to improve system coordination.
• Participates in orientation and training of Fred Hutch staff regarding patient education /patient navigation/coordination of services and cultural beliefs and practices of the population served.

Qualifications

Required:

• High School Diploma or equivalent
• Minimum of 2 years working in a hopsital or healthcare setting.
• Minimum of 2 years in patient advocacy, navigation, and/or coordination of care.
• Experience working with chronic disease management among Indigenous/tribal communities.
• Previous experience in tobacco education or cessation services.
• Familiarty and/or experience with the Indian Health Service (CHSDA) and government sponsored health care grants/initiatives for Indigenous communities.
• Experience in advocacy, outreach, coordination and collaboration with Indigenous communities and health care settings.
• Must have and maintain valid WA State Driver's License, valid insurance, and have clear driving record.
• Excellent interpersonal skills, written and verbal communication skills, and proficiency with
• Microsoft Office applications required (including Microsoft Outlook, Word, Excel, Power Point, Access and Visio). Attention to detail required, ability to troubleshoot or anticipate change desirable.
• Understanding of, and competence in, highly professional realm requiring diplomatic communications with internal and external groups, with effective fielding of inquiries and professional representation of the program.
• Works well independently with minimal supervision and is expected to use initiative and sound judgment in carrying out responsibilities within the framework of the department procedures.
• Must have a valid Washington Driver's license. Travel required: approximately 10%, may be required for overnight travel for meetings and events. Ability to work evenings and weekends
• Self-motivated team player, with demonstrated ability to work independently with minimal supervision. Diplomacy and understanding of political sensitivities. Tactful approach in handling confidential and sensitive information and a high sense of responsibility and discretion.
• Demonstrated understanding of medical terminology.
• Knowledge of health care and social practices in Indigenous communities.
• Experience and demonstrated ability in providing direct services to Indigenous families.

Preferred:

• AA Degree preferred - or 3 years relevant job experience in lieu of education.
• Tobacco Cessation

• Experience in advocacy, outreach, coordination and collaboration with Indigenous communities and health care settings.
• Established Indigenous/tribal community relationships.

The hourly pay range for this position is from $29.77 to $42.39 and pay offered will be based on experience and qualifications.

This role will have the opportunity to work partially at our campus and remotely.
